Where could I print out a repair form to send with my canon sd1000 to get fixed?

I've been searching all over the stupid website and they haven't given me crap. Just that estimate thing which is NOT what I need. Please help! Where can I get a PRINTABLE repairs form to send away with my canon sd1000 7.1 to be fixed?!
